The denials this specialty sees

The most common revenue loss is not a denial. It is billing 90460 once for a combination vaccine that contains several components, when 90461 should have been added for each additional one. Nothing denies. The claim just pays less than it should, and it does so at the volume a pediatric practice vaccinates at.

The counselling requirement is real and it applies. 90460 and 90461 require that a physician or other qualified professional counselled the family, and the note has to reflect it.

VFC billing is the second pattern. Program-supplied vaccine is billed with the administration code and the vaccine product reported at no charge. Billing the product as though it were purchased stock is a repayment problem rather than a denial.

Third is the well-child visit that turns into a problem visit. A child comes in for a check-up and has an ear infection. Both are billable, with modifier 25, when the note documents both. See CO-97.

Payer notes

Medicaid managed care rules vary substantially by state, including on well-child periodicity schedules and which vaccines the state supplies.

Some commercial payers do not recognize 90461 and pay a flat administration fee. Knowing which of your payers do which prevents chasing denials that are actually contractual.

Common questions

Are we billing vaccine administration correctly?

Check whether you are adding 90461 for each additional component in combination vaccines. Many practices bill 90460 alone. At pediatric volumes that gap is usually the largest single recovery available.

Can we bill a sick visit during a well-child check?

Yes, with modifier 25, when a separate problem was evaluated and documented. The well-child visit has its own required elements, and the problem work has to be visible on top of them.

How do we bill VFC vaccines?

Bill the administration code and report the vaccine product at no charge, following your state's convention. Billing program-supplied vaccine as purchased stock creates repayment exposure even though the claim pays.
